An old say states: You can not fry eggs without breaking it.
Of course some kids died. They can also die in the street or by ordinary illness. Of course it sound awful when they died from an infection, which could be fixed within minutes in a hospital from they are withheld due to suspicion of manipulation, but that is life. Of course dying from either starvation or dehydration is a awful way to die, but is it less painful to be run down by a car?
All manipulates - even Presidents when the situation calls for it. How do you think this great country is run? Expect manipulation. Even if you child talks about death. However: Be aware that the staff often also manipulate by recommending placement in another program, they happen to own. That is not wrong. They have a business to run.
Don't be fooled by false confessions. Many of these places have level system where it is normal for new kids to observe old kids. Here they learn what kind of confessions, they have to bring on the table in order to achieve privileges. You may question whether it is good to learn them how to lie, but think about their future. How will they survive in an adult workspace or in politics if they are always honest?
Before entering a seminar, remember who is paying for it. It is not you as a parent who has something to fix. It is your child and that is what you are paying for. If they insist on your participation, perhaps you have chosen the wrong program.
